- Incident Reporting
WSU SOM is committed to creating and fostering a campus environment free from all forms of discrimination and harassment. An "incident" is any event or conditions that threaten the safety and/or security of those involved and/or pose risk to individuals or to the institution, be it physical and/or emotional. The e-Kiosk is designed to provide our medical students with ready access to a wide range of key services.
- Begin by clicking on the appropriate form below.
- Complete the required fields in the form.
- Select the type of letter you are requesting and upload the appropriate supporting documentation needed for your letter. Do not submit the request until all documentation is attached. (Example, CV, Jury Summons, etc.)
- Click the submit button located at the bottom of the form and the request will be routed to the appropriate person. If further information is needed the letter writer will contact you.
- You must request your letter at least 10 days prior to the application due date or submission deadline.

These forms are to be completed by graduated MD students from the SOM. ERAS tokens or updated MSPE letters can be requested. If you are applying through ERAS for a fellowship, we do participate in the EFDO Medical Institution Document Service (MIDUS).

The medical school encourages you to refer to the Students with Disabilities (SDS) website if you have a documented disability or suspect you have a disability that will impact your medical school performance. The SDS office can be contacted at 313-577-1851. Office hours are Monday-Friday 8:30-5:00 with extended evening hours on Monday and Thursday evenings until 7:00 during the fall and winter.
