Thank you for choosing Pediatric Urology to provide your pediatric urology services. This brochure will familiarize you with our office staff and policies. If you have any question or concerns, please call us.

About Our Staff

We have chosen our staff for their training, experience, and knowledge. Our staff is always will and available to assist you with any problem or concern.

Appointments

We see patients in our office from 8:30 a.m. to 5 p.m., Monday through Friday. However, each physician has different office hours. Please call (313) 745-5588 during regular office hours to schedule an office appointment.

If you are unable to keep your scheduled appointment, we would appreciate a phone call 24 hours prior to that appointment, so we may schedule another patient in your place.

If an emergency occurs and your physician is not available, we will arrange for another physician to care for you.

Things to Bring for Your Appointment

In order for our physicians to properly diagnose and treat your child, it is very important that you bring all medical records, x-rays (actual x-ray or ultrasound films) and laboratory results relating to your child's urological problem. Unfortunately, our experience is that records mailed to us frequently do not arrive on time for the child's visit. We strongly recommend that you pick the records up and bring them to your visit.

Phone Calls

We have devised a phone system that will enable us to handle your call as promptly and efficiently as possible. Please listen to all the options and select the one that best fits your needs.

Our staff has been instructed to answer all incoming calls as quickly as possible and to handle emergency calls immediately.

If you wish to leave a message for the physician, our staff will be happy to take a message. The physician will return your call as soon as possible. This call may not be returned immediately because the physician may be in surgery and may need to review your child's chart before returning your call.

After Hours and Emergencies

If you have an urgent problem after office hours, dial (313) 745-5111 and ask the operator to contact the urologist on-call for Children's Hospital.

In the event of an emergency, go directly to Children's Hospital of Michigan Emergency Center or the nearest emergency center. Be sure to tell the staff that you are our patient so they will contact us.

Prescription Refills

If you are in need of a prescription refill, please contact our staff during regular business hours, when the physician can review your child's record. Please have your pharmacy phone number on hand when you call.

Insurance Policy

Our practice participates with many insurance companies. If you are a member of an HMO, please make sure you bring a referral form for each visit, x-ray, laboratory study, or surgery. Unfortunately, if you do not have a referral your insurance may not pay for your office visit. You will be responsible for full payment at the time service is rendered. A co-payment required by your insurance is due at the time of service. If your insurance does not cover office visits, full payment is expected at the time services are rendered.

Please be aware of your insurance coverage and what is covered. There are too many different insurance policies for us to know what is covered by each. If you have any questions, please contact your insurance carrier prior to your visit.

In addition, please advise our receptionist of any changes in the patient's name, address, phone number, insurance coverage and referring physician that may have occurred since your last visit.
