Duplicate Diploma

Request must be made in writing.  Please send request to the above address, which should include your name as you wish it to appear on the diploma, check or money order (made payable to Wayne State University) in the amount of $20 and complete mailing address for the duplicate diploma.  Diplomas cannot be mailed to a post office box.  

Grades and Evaluations

All grades and evaluations are collected for the student record and transcript purposes.  If you would like to review your grades and evaluations, please contact your counselor in the Office of Student Affairs.

Name/Address Change

To facilitate a change of address and/or phone number, submit the information to the Office and the change will be made on all University records.  Please remember to inform us promptly of all address and/or phone number changes.  We mail you critical, time sensitive material and it is very important for you to receive mailing a timely fashion. 

To facilitate a change of name on an academic record, a copy of the original court document (i.e., marriage license, divorce decree, etc.) is needed.  Submit documentation to the Office and the name will be changed on all University records.

Transcripts

Requests for transcripts must be made in writing with original signature and should be addressed to the Office.  Generally, please allow 2-5 working days for processing your request.  Note that the processing time may vary depending on the time of year and volume of requests, so it is advisable to send in your request 1-2 weeks in advance of your deadline.  Official transcripts are $2.00 per copy (check or money order made payable to Wayne State University).  Your request in writing should include the following information:

  • Full name used while in attendance
  • Your address and telephone number
  • Your graduation date
  • Your signature
  • The address to which transcript is to be mailed ($2 per copy)

Resident/Residence Classification

A non-resident student may apply for resident status if s/he believes that circumstances warrant it.  Resident status, if granted, would allow the student to enroll in future terms at resident tuition rates.   Forms are available in the Office.
