- One page summary of your goals:
- -Future plans after completion of the fellowship
- -The rationale for your pursuing fellowship training and other aspects that you may consider helpful in assessing your application
- Previous experience in research, whether this has led to a degree or not (e.g., PhD, MPH)
- The application deadline is August 1st,
- Registration for the specialty matching service of the National Resident Matching Program (NRMP)
- Satisfactory completion of an ABOG approved residency
- Michigan medical license
