Early Middle Cerebral Artery Infarction

CT of the brain demonstrates effacement of the right Sylvian fissure and parietal sulci as compared to the left. The gray-white matter interface is blurred on the right and there is subtle lower attenuation of the brain on the right.

 

 

 

 

 

 

 

Follow up CT 24 hours later shows diffuse low attenuation in the right parietal region with moderate midline shift to the left. The findings are compatible with infarction in the right middle cerebral artery distribution. Notice the sparing of the right frontal and occipital lobes which are supplied by the anterior cerebral artery and posterior cerebral artery, respectively.

 

 

 

 

 

 

 

 

In this case, craniotomy was performed to relieve pressure from the severe brain swelling. A ventriculostomy tube was also placed.
